Uniform Request

The following types of requests can be fulfilled using this form:

Initiate an exchange of a uniform part that no longer fits.

Request a replacement uniform piece for an item that is missing.

For any other requests, please speak to the Supply Officer.

Please provide more details. (Ex. Pants fit in the waist but are too short. Shirt is too tight at the neck.)
